Completing a hazard analysis and risk assessment (HARA) is one of the key steps in ensuring the item is safe. 

When completing the HARA, the first activity is the Hazard Analysis.  This is evaluating the malfunctioning behaviour of the item for causes of potential harm to people, and the operating conditions necessary to be present for the hazard to result in harm.  

ISO 26262-2:2018 clause 6.2 states ‘Hazard analysis, risk assessment and ASIL determination are used to determine the safety goals for the item’.  Therefore if hazard analysis is not completed adequately then there may be a hazard that is not prevented, because a safety goal is missing.  

This ebook gives guidance and a structure for completing this analysis, based on best practice seen in the automotive industry.   That is fully compatible and compliant with ISO 26262:2018 for the scope of the guidance.

The guidance:

  • defines what to do before the hazard analysis as inputs to the activity, to ensure that the hazard analysis can be adequately completed. 

  • provides a template for completing the activity,

  • describes how to complete the activity using this template following HAZOP methodology.

  • recommends phrasing for defining a hazard

  • includes a comprehensive list of hazard descriptions that is a compilation of hazards seen from reliable sources.  

  • provides a verification checklist to be used by the authors / and reviewers to ensure the hazard analysis is adequately completed. 

  • Includes a conformity statement - defining what ISO 26262:2018 clauses and objectives that are applicable to this guidance, and how they are met.
